UPDATES
Symptomatic Tier 3 patients (regardless of vaccination status) and all unvaccinated Tier 3 patients will be enrolled for 10-day follow-up (Tier 3 defined below).
Tier 3 Criteria
Any of the following:
- Age/Morbidity
- ≥ 75 years old
- < 75 years old with:
- CVD
- ESRD
- Immunocompromised
- Severe COPD,
- Cirrhosis
- Cancer
- Frailty or multiple comorbidities
- Severe Symptoms
- Cough with pain
- Wheezing or SoB >6 days
- DOE
- Chest Tightness
- Clinical Data
- O2 Sat <94%
- Infiltrates on CXR or CT chest
HOW TO REFER A PATIENT
- For Epic referrals:
- Tier 3 CVMP placed through HHP Complex Care referral to STRAUB
- Reason for referral: Enroll in CVMP
- Call the CVMP line at: (808) 462-5430 – Option 4
- Referrals via Primary Care, ED, Urgent Care and Respiratory Clinic
- Triaged by Complex Care and enrolled into CVMP or referred to PCP as appropriate
ABOUT
The Hawaii Health Partners COVID-19 Virtual Monitoring Program and Patient Registry Management Program are part of the the COVID-19 Population Health Virtual Care Team; their purpose is to assist providers in caring for COVID-19 positive patients.
- COVID-19 Virtual Monitoring Program: A 14-day program, which supports providers in monitoring the health of confirmed COVID-19 positive patients, and works with providers to facilitate patient care. Click here for the workflow algorithm.
- COVID-19 Patient Registry Management Program: This service is automatically applied to all patients that are tested for COVID-19 at any HPH site, including drive-up specimen collection site. Click here for the workflow algorithm. The purpose of the test registry management program is to ensure that all patients receive their test results in a timely manner so they can take immediate steps to care for themselves and help protect others in their home and community.

The Hawaii Pacific Health (HPH) COVID-19 Virtual Clinic for video and phone visits is open for our patients and the public. The purpose of the Virtual Clinic is to address patients with signs and symptoms of COVID-19 who are well enough to be seen without coming to the office and others who have concerns about COVID-19 exposure.
Patients will initially speak to a Registered Nurse, and if appropriate, the clinic will offer video and phone visits with a provider. The virtual team will recommend and facilitate pathways for care and order COVID-19 tests when clinically appropriate. HPH physicians, advance practice providers and registered nurses will staff the clinic.
