Chronic Kidney Disease (CKD) Education and Navigation - An Opportunity

Learning Objectives: At the conclusion, participants should be able to:

  • Utilize CKD GFR/albuminuria heatmap to better identify, risk stratify and educate CKD patients to allow informed shared decision making, improved outcomes and decrease health care costs.
  • Understand the sources of Kidney Disease Education in Hawai‘i.
  • Recognize Kidney Disease Improving Global Outcomes (KDIGO) as a source of CKD clinical practice guidelines.
