   For Specialists - Annual Assessment of Chronic Conditions: Friday, May 30

   The first presentation of 2025 for the “Annual Assessment of Chronic Conditions” Measure will be distributed on Friday, May 30. This measure aims to improve the understanding of risk-adjustment and its importance in describing disease complexity through educational sessions. Eligible members can earn HQIP credit by watching the presentation and completing the question set by the deadline on Monday, June 30. This measure is separate from the HHP Network Engagement – Webinars measure.

   2024 QPP/SPP Provider Review Period

   For All Providers –  2024 Quality Performance Program and Shared Savings Program (QPP/SSP) Provider Review Period: June 2 – June 30

   Final performance for the 2024 Hawai‘i Health Partners QPP/SSP (now HHP Quality Incentive Program) will be available on Monday, June 2, for your review. Eligible members, please review your performance through the HHP dashboard. Measures specific to your specialty and your performance in these programs will be reflected. We encourage you to review your report to ensure the accuracy of the information presented as this will be used in the calculation of incentive payments. You may submit any discrepancies along with supporting documentation to [email protected] before the closing date of this review period on Friday, June 30.

   Additional details will be provided in the June HHP Newsletter and in an email at the start of the review period
